Physically: Nothing worse that just sore wrists and the like.
Mentally: Played so much Pokémon Blue when I was ten that I was starting to hear it's theme tune even when I wasn't playing it or in an event nothing to do with Pokémon. It was all in my head.
Here's something of a trait of mine I notice when I game. After a while when I played to much, my ears go all red. No other part of my body does that, just my ears.

The ends of my thumbs would hurt so much that I had to make special prosthetic thumb covers for myself so that I could keep playing... My eyes would turn red as all the little capillaries burst, (that STINGS so much!). I’d get terrible migraines that would leave me in bed for a whole day and a night –terrible pain in my head, nausea, vomiting. I couldn’t close my eyes at night without everything I thought of being rendered like an early 90’s anime themed fantasy RPG.

HA-ha... that happend to me also during the NES and SNES days. I put layer and layer of band-aids to protect my thumbs.

oh yeah. I used to get blisters on my big thumbs playing SNES games of all kinds.
actually at times I still get that playing on my playstation 2 if I aren't careful and get competely deep into the game itself.
and one time I got sores on my wrists and fingers playing clock tower for SNES on the emulator.

Never hurt myself playing DDR or ITG, but I have badly injured my hand playing SSBM.
Basically, I picked up the habit of slamming the handle into my palm every time I died.
It started hurting after a while.

I once popped my thumb out of place in the arcade. I forget what it was I was playing, it was some fighting game. Years before, I'd broke my thumb in a stunt man accident, and it popped over the wrong way because I jammed down to hard. I had money on the game, so I couldn't say anything, I had to beat the guy I was fighting, then worry about my thumb. I'm pretty sure I beat that guy, since I don't lose at video games. Even with a bone fracture, I can still kick ass. It was a fighting game, maybe Tekken 4 or 5. It was about a year ago.

When Street Fighter 2 first came out for the SNES, my friend Nathan and I played until we wore away the top layers of skin on our thumbs, which turned into huge blisters. Then we put Band-Aids on them and kept playing.
I'm also doing that other comic "Space Waffles ".
We now have a podcast called The Random Pirate Comics Show!

Pretty much every video game system I have has fallen on me at some point or another. My friend Anna once strangled me during Harvest Moon, for reasons I can't quite remember. She punched me every time I beat her in Super Smash Bros and she hit me over my head with a controller for talking during a cut screen. I think that's more related to her personality than to video games, though.

this happened one time when my friend was sleeping over:
We had decided to stay up all night and my friend went to sleep while I was playing dragon warrior. I fell asleep and fell off my bed.
I landed on a glass and broke it with my shoulder. I didn't really hurt myself, It did hurt though.

move is never a good idea that mat slide out from under my feet and I hit my head on a coffee table was out for a couple of hours.
My friend is one of those people who swings his body while playing games. One time he Punched me in the face while playing Halo 2 at a LAN.
Other than that just blisters, hurt knees, hurt plams (hit a crappy control against my plam), and cramps thumbs.

Video game injuries, hmm?
I've got a big list, but i'll just post some of my highlights here:
-When I was 6, I was playing Super Mario, and I fell sideways and got my head lodged between a desk and the wall
-I got electrocuted slightly during a thunderstorm while playing some ninja game.
-Everytime I beat my friend in Halo, Mortal Kombat, etc., He would hit me really hard. after a while, it really started to add up.
-every time I play with my brother, we yell at each other about cheap shots or stealing each other's supplies.
one time he walked away with a bite mark and a black eye, while I got a broken nose
-one time, while playing Pool (i know it isnt a video game, but this one is funny.) my friend hit the ball, sending it leaping off the table directly into my groin.
-After playing Eternal Darkness: Sanity's Requiem, Condemned: Criminal Origins, And F.E.A.R., I felt as if my computer was possessed and plotting to kill me, as well as being thoroughly afraid of dark places for a good few weeks.

When I was 10, I beat the Legend of Zelda: Link to the Past... so decided to try and make it more challenging, so I'd hang upside-down using the top bunk of me and my brother's bunk bed.
One day, I was playing on Turtle Rock (yes... I got THAT far... upside down) and my brother pushed my feet off the edge, and I fell and hit my head.

When I was playing House of the Dead III Arcade machine, there was a one time where I had to aim it through the "sights". I kept on pumping lead and the next thing I knew my arm started to hurt like hell.
And I still died in the game. Damn crazy sloth.
Also, does the heavy weight from Time Crisis guns count as well?
Edit: There was also one time where I just collected all 100 hidden packages in Grand Theft Auto III. Before saving, I thought I'd also do the fire missions before quitting. When I did the fire mission trick (Where you start the replay and the fire gets put out automatically), the game froze.
The game goddamn froze.
After force-closing the game, I started banging my hand on the wheelchair arm so hard that... well, it hurts. I put a lot of effort trying to find them all. There goes all the hidden packages of Northern-Half of the Shoreside Vale.
And I still died in the game. Damn crazy sloth.

I was playing mario party 4 (I believe) when we were doing the one where you have to escape from the ghost on skateboards. My nail got caught in the A button. I am not kidding.
For another humilating story, I was doing DDRE on standard where I ran up against an impossible combination. Upon attempting to do it I found out why they say wearing socks while playing was not a good thing. I fell backwards onto the dog. He didn't bite me as much as he blasted out my eardrums by a pure doggy-shriek of fright.
As a sidenote I was playing Battlefront on a wireless while standing on my couch. I don't think I need to say anything else on the matter. XD
