At least a few times every day I've been playing Smash TV. Why? To get the "No Continues" achievment. The problem? For me at least, it's ALMOST IMPOSSIBLE!!! Smash TV was a game that was made to be two things... to be so fun to play that you don't want to stop, and to be so hard that it'll suck that ten dollar quarter roll out of your pocket really fast... and this was way back when games only cost a quarter to play!
Of course on XBox Live you can play the game with as many continues as you want, but in order to get that achievment you can't continue at all. Also, if you do continue your XBLive score is divided by the number of continues you used. So you could have millions of points in the arcade, but if you continued a lot that number could be whittled down to just a few thousand.
Well, enough of this. Back to my attempt at getting that near impossible achievement (this may take a long while...) T_T

One of those nearly impossible achievements that I know of are the companion achievements in Mass effect. Essentially, if you went through the entire game with the same companions, you got couple of X-box-points (or E-penis points as I like to call them) and that character became slightly better or something like that. Problem was that it often didn't work and many complained that it was impossible to get them with the companions that you got later in the game. The only way to get them was to actually buy the two expansions and play through them with those companions as well.
Normally I don't give a rats ass about those achievements. Especially if they're incredibly redundant and stupid. The game developers have gotten a bit better at making achievement hunting more fun though or at least more intergrade with the game and I liked how Mass Effect gave you secondary bonuses, like unlocking additional skill trees, granting damage bonuses with weapons you use allot and faster level ups with future characters.

I'm gonna vote for getting the canon ending in "Dead Rising". No, it's not impossible, but it requires saving every survivor in the game. This wouldn't be so difficult if the survivor AI didn't suck. I love the game otherwise, but the fact that going for the canon ending turns the entire game in a string of escort missions with survivor AI that can rival the zombie AI has made me quit the game many, many times. And I know I'm not the only one who's gotten that frustrated with it.
I swear a number of the people who have gotten the canon ending on the XBox version either have WAY too much time on their hands (dying means you either start at a checkpoint or you start over with your stats and skills intact, and having even one survivor die means you have to start from one of those two places) or had a run of dumb luck.
As for the impossible acheivement in the game, answering all of Otis' calls. He calls you very, very often and when you answer you can't run or attack. So, yeah, answering the walkie-talkie means that you're wide open to attack and getting hit makes you hang up. And he'll call you back right away, berate you for being rude (well, SORRY for getting sliced up by the chainsaw-wielding maniac) and then start the original message over from the beginning. He also calls no matter what's happening, so if you need to fend off a horde of zombies in order to safely answer and Otis hangs up in the meantime you've lost your chance at the acheivement. And for a guy who apparently watches the security camera monitors 24/7 he sure likes calling when you're in the middle of fighting for your life.
Capcom, there's making a game a legitimate challenge and then there's artificial difficulty. One involves things like puzzles that challenge the player's intellect, the other involves things like poor programming.

I was under the impression that you simply can't save everyone in one playthrough, simply because saving a few specific survivors means that a few others will inevitably die. But saving the maximum number of survivors in a single playthrough is a work of strategy and timing, and maybe a little luck.
By the way, I've never had more fun with achievements that when I was playing Dead Rising. Each one was a fun new challenge. But then, it really does depend on how much you enjoy the core game in the first place, and I really enjoyed it, despite the game's weaknesses.
But you're right, the Otis achievement is too hard and irritating. It's one of the few I don't have.

I'm not saying I didn't enjoy the game. I love the combat, the story is fun in a campy way (seriously, how do you end up with zombifying wasps when you're trying to breed cows?) but it's the AI that gets to me after a while. If the other survivors were just a little bit smarter I'd put the game on my personal list of "best games ever". Hopefully that particular issue will be fixed for the sequel, although I will miss Frank's camera.

It leads into Overtime Mode which, on completion, leads into the truly sandbox survival mode on the 360 version where your only goal is to try to survive both the zombies and starvation for 7 in-game days.

I found the one acheivement in Watchmen: The End Is Nigh, where you have to go through a level without getting hurt once.
